Martin is going through a terrible bout of depression, it's not even brought on by his circumstances, not him being miserable, though the situation doesn't help any. He finally makes the decision to end his own life, and goes through the motions of putting his affairs in order and tying up loose ends with a sort of numbing calm.
One of the things he decides to do, is to tell Douglas how he feels about him, convinced 100% that the man does not reciprocate at all, and not waiting to hear the other's reply. (Whether Douglas does reciprocate or not would be up to the author, though I'd prefer it ended D/M, but that Douglas hadn't been pining for Martin, or aware of any feelings for him)
Martin survives the attempt, and Douglas feels incredibly guilty, thinking he is to blame for the attempt.
